Family Physicians/ Primary Care providers can continue to refer patients using the referral process, as referrals are continuing to be triaged and processed.
The Musculoskeletal Central Intake and Assessment Centre (MSK CIAC) will provide you with a timely assessment of your hip, knee or low back pain condition.
The MSK CIAC is one point of contact for hip, knee and low back pain referrals in the HNHB LHIN (Hamilton, Niagara, Haldimand and Brant Local Health Integration Network). We see adults with moderate to severe osteoarthritis of their hip or knee, and adults with low back pain.
The HNHB LHIN includes Hamilton, Niagara, Haldimand, Brant, Burlington and most of Norfolk county.
Primary Care Providers can refer patients to MSK CIAC using the standardized hip and knee, and low back pain referral forms.
Patients are assessed within four weeks by an Advanced Practice Provider (Physiotherapist or Chiropractor) with specialized skills and training. The Advanced Practice Provider will discuss treatment options with you and come up with a personal plan that is right for you. This helps to reduce the time it takes to see a specialist, begin treatment, and if necessary, have surgery.
Patients are fully covered by OHIP.
