The MSK CIAC Low Back Pain Program is designed to provide patients with timely assessment of their low back pain condition, and a personal self-management plan that will allow them to manage their low back pain.
MSK CIAC is one point of contact, in Ontario Health (West), for referrals for adults with low back pain. Ontario Health (West) includes Hamilton, Niagara, Haldimand, Burlington and Brant Region.
Primary Care Providers or nurse practitioners can refer patients to the MSK CIAC for low back pain and/or low back related leg pain, to determine next steps in their care. Referral to the program is only available to patients whose Primary Care Provider or nurse practitioner has enrolled in the program.
The MSK CIAC Low Back Pain Program is for patients who are 18 years of age and over and:
Patients are assessed within four weeks by an Advanced Practice Provider (Physiotherapist or Chiropractor). The Advanced Practice Provider has special training in low back pain.
Assessments take place in MSK CIAC Low Back Pain Assessment Centres. The MSK CIAC Low Back Pain Program has 14 Assessment Centre locations in communities across the region. Patients will be booked for an assessment at the location closest to their home.
Patients are fully covered by OHIP.
