Referral

A referral for assessment for primary hip or knee arthroplasty can be faxed to MSK CIAC using the MCK CIAC hip and knee referral form. We do not accept consult notes or any other referral forms.

Please note the special x-ray requirements on the referral form.

Patients must come to their appointment with a CD or film x-ray of their affected joint taken within the last 6 months. An MRI is not required. If the patient images are not available to be reviewed, the appointment will be rescheduled.

*Please note – If the x-ray was taken at a hospital in Hamilton, Burlington, Niagara, Haldimand Norfolk, Brant or Burlington you do not need to bring a CD or film x-ray.

Hip and knee assessments will take place at Assessment Centres located in Hamilton, Brantford, Burlington and Niagara. Patients will be booked for an assessment at the location closest to their home, or they may choose the first available assessment at any of the locations. Please indicate on the referral form.
